Home Health Aide (Per Diem)

The home health aide provides personal care and related services in the home that are allowed to be performed under state law. They function under the direction, instruction, and supervision of the staff nurse, Chief Nursing Officer, and/or appropriate supervisor. 

Tasks to be performed by an HHA must be assigned by and performed under the supervision of a Registered Nurse who will be responsible for the patient care provided by the Home Health Aid. 

Registered Nurse (Per Diem)

A professional who provides professional nursing services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates patients and their families. Professional skilled nursing is performed under the supervision of the Chief Nursing Officer

Licensed Practical Nurse (Per Diem)

A Licensed Practical Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors, teaches and educates patients and their families according to the established professional plan of care. Licensed Practical nursing is performed under the supervision of the assigned RN’s, Administrator, and Chief Nursing Officer.

Intake & Scheduling Coordinator (Full-Time)

Intake & Scheduling Coordinator manages patient referrals, coordinates care schedules, and ensures seamless service delivery. This role is key in matching patients with the right care professionals, maintaining continuity of care, and supporting communication between families, staff, and healthcare partners. You’ll also contribute to quality assurance and help improve operational processes.
